Request claim forms by mail. Please refer to your Health Plan member ID card for the correct mailing address. Do not fax claims.

Your have a right to appeal claims decisions. If you believe we violated our claims or appeals procedures, or that our procedures are deficient, you may appeal to OPM.

Request prior approval or precertification

To prevent your benefits from being reduced, request prior approval or precertification for types of medical care listed in Section 3 of your Plan brochure. Call UnitedHealthcare 24/7 at least two business days before admission or before receiving services:
