Get benefits information or check the status of a claim by calling our member services number to access self-service options 24/7.
If you have coverage questions, call member services or submit our online form and receive a response within 48 hours.
Submit a medical claim to request reimbursement for care.

Request claim forms by mail. Please refer to your Health Plan member ID card for the correct mailing address. Do not fax claims.
Your have a right to appeal claims decisions. If you believe we violated our claims or appeals procedures, or that our procedures are deficient, you may appeal to OPM.
