How to Enroll in the APWU Health Plan


High Option Benefits Information
 
Postal employees

Complete a Health Benefits Registration Form (SF 2809) by calling PostalEASE at 877-477-3273, and following the prompts. You must have a Personal Identification Number (PIN) in order to enroll. You can also enroll on the PostalEase Employee Web on the Intranet (from the blue pages) if you have access. For questions about either way of enrolling, contact your local personnel office.

 
Consumer Driven Health Plan Benefits Information
 

Non-Postal employees/annuitants

To choose the APWU Health Plan, go to your Personnel Office or see your local Health Benefits Officer and request a Standard Form 2809 (SF2809). You can also access an SF 2809 by clicking on this link: www.opm.gov/forms/pdf_fill/sf2809.pdf.  Print a copy, and provide to your Personnel Office or your local Health Benefits Office. Electronic and telephonic enrollment is available in some agencies.

The APWU Health Plan Enrollment Codes are:

    471 Self Only - High Option
    472 Self and Family - High Option
    474 Self Only - Consumer Driven Health Plan
    475 Self and Family - Consumer Driven Health Plan
