Manage your claims
Submit a medical claim to request reimbursement for care.

Request claim forms by mail. Please refer to your Health Plan member ID card for the correct mailing address. Do not fax claims.
Your have a right to appeal claims decisions. If you believe we violated our claims or appeals procedures, or that our procedures are deficient, you may appeal to OPM.

Children up to age 26 are eligible for health coverage under a parent’s Self & Family enrollment:
- Married children are allowed to be covered
- There are no dependency or residency requirements
- A child is not required to be a student
- A child is not required to have prior or current insurance coverage
